During this event, Luxe Wellness may capture photographs and videos for digital mediacontent & marketing purposes. This form seeks your consent to use such images and videos.

I do not grant permission to Luxe Wellness to use any photographs or videos in which I am identifiable. Please ensure that I am not included in any captured media.

1. I understand that by giving my consent, Luxe Wellness acknowledges that any photographs or videos in which I am identifiable will be used responsibly and ethically, respecting my privacy at all times.

2. I am aware that this consent can be withdrawn at any time by contacting Luxe Wellness at brooke@luxewellness.com.au, but that withdrawal will not affect the use of materials created prior to the withdrawal.

3. Luxe Wellness agrees to not use these images or videos for any purpose beyond those explicitly mentioned without further consent.
